Room Request
This form is for UUSG events only. Outside groups, please contact the office at 630-232-2350 or office@uusg.org for information about using our facilities.
This form must be filled out in order to reserve a space on the calendar and in the building.
After submitting your request, you will be contacted via email or phone to confirm your reservation.
We will do our best to accommodate your requests, but please understand that we must occasionally change room assignments to allow for the best use of our limited space.
